2024 Form 65 Branch Membership List

Please send in your branch's Form 65 Member List by April 30. Email it to Form65Recipient@sirinc.org and to your Area Representative. A template and instructions are here. If you can't send a spreadsheet, send the minimum list of required contact information in whatever way you can.

Less Paperwork for Branches

Membership status can be reported to State SIR on the new Form 28 Cash Report, so branches do not need to submit a separate branch membership status form each month. Branches may use Form 27 for reporting membership status to their BEC.

2024 Branch Directory

Check out the new directory of SIR branches and leaders. Verify that your branch's officer information is up to date so they can stay in the know. If an update is needed, your branch secretary should send it in a Branch Leadership Update Form 20.
